Homologación lingüística al idioma español, en población chilena, de la herramienta Sunnybrook Facial Grading System para evaluar parálisis facial. Estudio piloto

Resumen Introducción: La parálisis facial es una patología muy común. La escala "Sunnybrook Facial Grading System" (SFGS) se ha posicionado como una herramienta útil y confiable para su evaluación y evolución. Objetivo: Homologar lingüísticamente desde el idioma inglés al español la escala SFGS en una muestra de población chilena. Material y Método: Tres kinesiólogos chilenos con dominio comprobado del idioma inglés tradujeron la escala SFGS al español. Un comité creó una primera versión de la SFGS en español. Posteriormente, un profesional del Instituto Chileno-Británico tradujo la primera versión nuevamente al inglés (retrotraducción). El comité definió la segunda versión de la SFGS. Finalmente, los investigadores llevaron a cabo los pilotajes. Resultados: En dos pruebas piloto, veinte sujetos respondieron correctamente el total de las expresiones solicitadas. Conclusión: Esta versión de la escala SFGS homologada lingüísticamente al esp añol puede ser aplicada a la población chilena.
Abstract Introduction: Facial paralysis is a very common pathology. The Sunnybrook Facial Grading System (SFGS) scale has positioned itself as a useful and reliable tool for its evaluation and follow up. Aim: To linguistically homologate the SFGS scale in a sample of the Chilean population from English to Spanish. Material and Method: Three Chilean kinesiologists with English proficiency translated the SFGS scale into Spanish. A committee developed a first version of the SFGS in Spanish. Subsequently, a professional from the Chilean-British Institute translated the first version back into English (back-translation). The committee defined the second version of the SFGS. Finally, the investigators carried out the pilots. Results: In two pilot tests, twenty subjects correctly answered the total of the expressions requested. Conclusion: This version of the SFGS scale linguistically homologated to Spanish can be applied to the Chi lean population.

Valores referenciales de posturografía basada en Nintendo Wii en población chilena

Resumen Introducción: La posturografía es un método de exploración complementario para valorar el grado de desplazamiento del centro de gravedad (COP); no debe usarse de forma aislada al evaluar equilibrio, pero se acepta para seguimiento de la respuesta a tratamiento. Es de baja disponibilidad, por su alto costo comercial. Como alternativa se documenta la plataforma Wii Balance Board (WBB) de Nintendo para registro posturográfico. Objetivo: Describir valores de normalidad en parámetros posturográficos en población chilena sin patología vestibular, usando un registro simple y accesible. Material y Método: Estudio transversal de valores referenciales en herramienta para evaluación del equilibrio. Se registraron valores posturográficos con versión adaptada del Sensory Organization Test (SOT) a adultos sanos. Mediciones con software WBB Sway Program mediante WBB. Se midieron índices somatosensorial, visual y vestibular, y patrones de control postural en ejes anter oposterior y mediolateral durante SOT-adaptado. Resultados: Se obtuvieron valores de velocidad de desplazamiento del COP, área del COP y desviaciones estándar correspondientes para 4 situaciones del SOT-adaptado de 35 sujetos entre 18-65 años, 21 mujeres, 14 hombres. Discusión: WBB permitió evaluar estos parámetros en población sana, como aproximación a determinar rangos de referencia. Esta herramienta de uso rápido y accesible constituye una alternativa útil para determinar patrones de control postural en sujetos sanos. Presentamos valores a utilizar como rango referencial en nuestra población; se sugiere utilizar estos valores como objetivo terapéutico en rehabilitación vestibular en pacientes evaluados en conjunto con otros parámetros clínico-sintomáticos.
Abstract Introduction: Posturography is a complementary method for evaluating the center of pressure (COP) displacement; it should not be used in isolation when assessing balance, but it is accepted for fo llow-up of the treatment response. Due to its high commercial cost and low availability, we documented the Nintendo Wii Balance Board (WBB) as an alternative for posturography. Aim: To describe the normal range of posturography parameters in the Chilean population without vestibular pathology, using a simple and accessible registry method. Material and Method: Cross-sectional study of referential values in a balance assessment tool. Posturography values were registered using an adapted version of the Sensory Organization Test (SOT) on healthy adults. Measurements with WBB Sway Program software through WBB. With adapted-SOT, somatosensory, visual and vestibular indices, as well as anterior/posterior and mediolateral axes postural control patterns, were measured. Results: Values of displacement velocity of the COP, COP area and corresponding standard deviations were obtained for 4 SOT-adapted situations from 35 subjects between 18-65 years, 21 women, 14 men. Discussion: As an appr oach to determine referential range values, WBB allowed to evaluate these parameters in healthy population. To determine healthy subjects postural control patterns this quick and accessible tool is a useful alternative. We present referential range values to use in our population; it is suggested to use these values as therapeutic objectives in vestibular rehabilitation in patients evaluated along with other clinical-symptomatic parameters.

Adherencia al uso de audífonos en adultos mayores con hipoacusia: Un ensayo clínico aleatorizado para evaluar un programa de rehabilitación auditiva

Resumen Introducción: La presbiacusia es una condición muy prevalente. Habitualmente se implementan audífonos como tratamiento, pero un problema es la baja adherencia exhibida que puede rondar un 50%. Objetivo: Evaluar si el programa de rehabilitación auditiva Active Communication Education (ACE) mejora la adherencia al uso del audífono. Material y Método: Se realizó un ensayo clínico aleatorizado controlado para evaluar la efectividad del programa ACE sumado a un refuerzo teleeducativo para mejorar la adherencia al uso del audífono. El proyecto fue aprobado por comité de ética y cada participante firmó consentimiento informado. La adherencia se evaluó por medio de la escala CIRUA y Dr. Yueh. Cada participante fue seguido en su domicilio en controles cada 3 meses hasta 1 año posterior a la intervención por encuestadores entrenados. Resultados: Se incluyeron 202 participantes, 101 en cada rama. La mediana de edad fue de 78 años y el 59,9% de la muestra fue de sexo femenino. El PTP promedio óseo fue de 48,59 dB y el aéreo fue de 55 dB. El grupo control tuvo adherencia de 65,91% y 62,30%. El grupo intervencional tuvo una adherencia de 78,41% y 72,28%. Hubo diferencias significativas en el porcentaje de adherencia entre grupo control y experimental según CIRUA (p = 0,027) y Yueh (p = 0,011). Conclusión: El programa de rehabilitación auditiva ACE es eficaz para mejorar la adherencia al uso del audífono en pacientes adultos mayores con hipoacusia. Este estudio representa uno de los mayores esfuerzos publicados para evaluar la efectividad de los programas de rehabilitación auditiva en adultos mayores con hipoacusia.
Abstract Introduction: Hearing loss associated with older adults is a very prevalent condition. Hearing aids are usually implemented as treatment, but there is low adherence exhibited, which can be around 50%. Aim: To assess whether the Active Communication Education (ACE) hearing rehabilitation program improves adher ence to hearing aid use. Material and Method: A randomized controlled clinical trial was carried out to evaluate the effectiveness of the ACE program added to a tele-educational reinforcement to improve adherence to hearing aid use. The project was approved by the ethics committee of the institution and each participant signed an informed consent. Adherence was evaluated using the CIRUA scale and Dr. Yueh scale. Each participant was followed at home in controls every 3 months up to 1 year after the intervention by trained interviewers. Results: 202 participants were included, 101 in each branch. The median age was 78 years and 59.9% of the sample was female. The average bone PTA was 48.59 dB and air PTA was 55 dB. Adherence in the control group was 65.91% and 62.30%. In the interventional group, adherence was 78.41% and 72.28%. There were significant differences in the percentage of adherence between the control and experimental groups according to CIRUA (p = 0.027) and Yueh (p = 0.011). Conclusion: The ACE auditory rehabilitation program improves adherence to hearing aid use in older adults with hearing loss. This study represents one of the largest published efforts to evaluate the effectiveness of auditory rehabilitation programs in older adults with hearing loss.

Reflejo olivococlear contralateral y su relación con ansiedad y calidad de vida en pacientes con tinnitus

Resumen Introducción: La presencia de tinnitus se asocia a un incremento del nivel de ansiedad y empeoramiento de la calidad de vida, sin embargo, aún no existe claridad de si el sistema eferente auditivo podría influir en estas relaciones. Objetivo: Determinar si las amplitudes de las emisiones otoacústicas por producto de distorsión (EOAPD) y el efecto supresor del reflejo olivococlear gatillado con ruido contralateral (ROC) se asocian al grado del impacto psicoemocional y nivel de ansiedad en pacientes con tinnitus. Material y Método: Se evaluaron las amplitudes de EOAPD y magnitud del ROC para cada oído de manera independiente en una cohorte de 32 sujetos tinnitus y 26 controles, cuyos valores fueron correlacionados con State-Trait Anxiety Inventory (STAI) y Tinnitus Handicap Inventory (THI). Resultados: Los pacientes con tinnitus tuvieron una correlación significativa entre la magnitud del ROC del oído izquierdo y los puntajes en las pruebas de STAI y THI. Co nclusión: Una mayor carga de ansiedad y peor calidad de vida en sujetos con tinnitus puede estar asociada con un empeoramiento del efecto supresor del ROC de la vía eferente auditiva.
Abstract Introduction: Tinnitus is associated with an increase in the degree of anxiety and with worse quality of life. However, whether there are relations between the function of the auditory efferent system and anxiety and tinnitus distress levels is not known. Aim: To determine possible associations between the distortion product otoacoustic emission (DPOAE) amplitudes and the suppressing effect of the olivo-cochlear reflex (OCR) with anxiety and psycho-emotional impact in tinnitus. Material and Method: DPOAE amplitudes and OCR strength were evaluated for each ear independently in a cohort of 32 tinnitus and 26 controls, whose values were correlated with the State-Trait Anxiety Inventory (STAI) and Tinnitus Handicap Inventory (THI). Results: A significant correlation of STAI and THI scales with the OCR strength measured in the left ear was found in subjects with tinnitus. Conclusion: A higher anxiety load and worse quality of life in subjects with tinnitus may be associated with an impaired olivocochlear reflex suppressing effect.

Differentiated thyroid carcinoma in children: A retrospective analysis of 125 pediatric cases from a single institution in India

ABSTRACT

BACKGROUND: Thyroid carcinoma (TC) is extremely rare in children. We assessed the clinicopathological features, outcomes, recurrence pattern, and associated risk factors of differentiated thyroid carcinoma (DTC).

METHODS: Children aged ≤14 years, pathologically diagnosed as DTC at a tertiary cancer institute between January 1998 and December 2015 were retrospectively analyzed. Survival outcomes were estimated using the Kaplan-Meier method.

RESULTS: During 18 years, 125 children with DTC were treated with a male:female ratio of 1:2.3. The median age was 12 years (2-14 years). Anterior neck swelling was the commonest presentation (72.8%). Histopathology revealed papillary thyroid carcinoma (PTC) in 123 children (98.4%). Extrathyroidal extension was seen in 32 children (25.6%). Sixty-eight children (54.4%) had nodal metastases and seven had distant metastasis. Relapse developed in 12 children. All were salvaged with subsequent surgery and radioiodine therapy. Eight children had persistent disease and one had a second malignant neoplasm. The median follow-up period was 9 years 1 month (1-20 years). Five-year recurrence-free survival (RFS) was 94.8% and 5-year overall survival was 100%. Larger tumors (p-value = .001), extrathyroidal extension (p-value = .001), and nodal metastasis (p-value = .022) were significant predictors for RFS in univariate analysi s.

CONCLUSIONS: Pediatric DTC showed aggressive behavior characterized by a high rate of extrathyroidal extension and nodal and pulmonary metastasis. Persistent disease should be distinguished from recurrent disease as DTCs with metastatic disease remain stable for long time and usually respond well to radioiodine therapy. Our study reaffirmed favorable prognosis despite aggressive presentation and even after relapse.

Impact of thyroid cancer treatment on assisted reproductive technology outcomes in women with infertility

ABSTRACT

PURPOSE: We investigated the effect of different surgical procedures and radioactive iodine treatment (RAIT) on in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) outcomes and evaluated whether possible risk factors, including age, thyroid-stimulating hormone (TSH) levels, and thyroid antibody positivity, were associated with adverse IVF/ICSI outcomes.

METHODS: This retrospective study included 76 women with infertility who had received thyroid cancer (TC) treatment among 137,698 infertile women who underwent IVF/ICSI cycles at the Peking University Third Hospital between 2010 and 2019. Clinical pregnancy and live birth rates were assessed.

RESULTS: We found that the clinical pregnancy and live birth rates in women who underwent partial thyroidectomy were 7- and 6-fold higher, respectively, than those in women who underwent total thyroidectomy. We observed no significant differences in the clinical pregnancy and live birth rates between the RAIT and non-RAIT groups, even after adjusting for age, TSH levels, surgical treatment, and thyroid antibody positivity. Multivariate logistic regression analysis showed that age and TSH levels were not associated with decreased clinical pregnancy and live birth rates. Women with thyroid antibody positivity had significantly lower clinical pregnancy and live birth rates than women without thyroid antibody positivity.

CONCLUSION: Our study showed lower clinical pregnancy and live birth rates in women who underwent total thyroidectomy than in women who underwent partial thyroidectomy. Thyroid antibody positivity is an important risk factor for adverse IVF/ICSI outcomes in women who have received TC treatment.

Brain microstructural and metabolic alterations detected in vivo at onset of the first demyelinating event

Abstract
In early multiple sclerosis, a clearer understanding of normal-brain tissue microstructural and metabolic abnormalities will provide valuable insights into its pathophysiology. We used multi-parametric quantitative MRI to detect alterations in brain tissues of patients with their first demyelinating episode. We acquired neurite orientation dispersion and density imaging [to investigate morphology of neurites (dendrites and axons)] and 23Na MRI (to estimate total sodium concentration, a reflection of underlying changes in metabolic function). In this cross-sectional study, we enrolled 42 patients diagnosed with clinically isolated syndrome or multiple sclerosis within 3 months of their first demyelinating event and 16 healthy controls. Physical and cognitive scales were assessed. At 3 T, we acquired brain and spinal cord structural scans, and neurite orientation dispersion and density imaging. Thirty-two patients and 13 healthy controls also underwent brain 23Na MRI. We measured neurite density and orientation dispersion indices and total sodium concentration in brain normal-appearing white matter, white matter lesions, and grey matter. We used linear regression models (adjusting for brain parenchymal fraction and lesion load) and Spearman correlation tests (significance level P ≤ 0.01). Patients showed higher orientation dispersion index in normal-appearing white matter, including the corpus callosum, where they also showed lower neurite density index and higher total sodium concentration, compared with healthy controls. In grey matter, compared with healthy controls, patients demonstrated: lower orientation dispersion index in frontal, parietal and temporal cortices; lower neurite density index in parietal, temporal and occipital cortices; and higher total sodium concentration in limbic and frontal cortices. Brain volumes did not differ between patients and controls. In patients, higher orientation dispersion index in corpus callosum was associated with worse performance on timed walk test (P=0.009, B = 0.01, 99% confidence interval = 0.0001 to 0.02), independent of brain and lesion volumes. Higher total sodium concentration in left frontal middle gyrus was associated with higher disability on Expanded Disability Status Scale (rs = 0.5, P=0.005). Increased axonal dispersion was found in normal-appearing white matter, particularly corpus callosum, where there was also axonal degeneration and total sodium accumulation. The association between increased axonal dispersion in the corpus callosum and worse walking performance implies that morphological and metabolic alterations in this structure could mechanistically contribute to disability in multiple sclerosis. As brain volumes were neither altered nor related to disability in patients, our findings suggest that these two advanced MRI techniques are more sensitive at detecting clinically relevant pathology in early multiple sclerosis.

Dual mTORC1/mTORC2 inhibition as a Host-Directed Therapeutic Target in Pathologically Distinct Mouse Models of Tuberculosis [Experimental Therapeutics]

Efforts to develop more effective and shorter-course therapies for tuberculosis have included a focus on host-directed therapy (HDT). The goal of HDT is to modulate the host response to infection, thereby improving immune defenses to reduce the duration of antibacterial therapy and/or the amount of lung damage. As a mediator of innate and adaptive immune responses involved in eliminating intracellular pathogens, autophagy is a potential target for HDT in tuberculosis. Because Mycobacterium tuberculosis modulates mammalian target of rapamycin (mTOR) signaling to impede autophagy, pharmacologic mTOR inhibition could provide effective HDT. mTOR exists within two distinct multiprotein complexes, mTOR complex-1 (mTORC1) and mTOR complex-2 (mTORC2). Rapamycin and its analogs only partially inhibit mTORC1. We hypothesized that novel mTOR kinase inhibitors blocking both complexes would have expanded therapeutic potential. We compared the effects of two mTOR inhibitors: rapamycin and the orally available mTOR kinase domain inhibitor CC214-2, which blocks both mTORC1 and mTORC2, as adjunctive therapies against murine TB, when added to the first-line regimen (RHZE) or the novel bedaquiline-pretomanid-linezolid (BPaL) regimen. Neither mTOR inhibitor affected lung CFU counts after 4-8 weeks of treatment when combined with BPaL or RHZE. However, addition of CC214-2 to BPaL and RHZE was associated with significantly fewer relapses in C3HeB/FeJ compared to addition of rapamycin and, in RHZE-trea ted mice, resulted in fewer relapses compared to RHZE alone. Therefore, CC214-2 and related mTOR kinase inhibitors may be more effective candidates for HDT than rapamycin analogs and may have the potential to shorten the duration of TB treatment.

In Vitro Activity of Oxazolidinone against Nontuberculous Mycobacteria, Including Macrolide-Resistant Clinical Isolates [Susceptibility]

We evaluated the in vitro activities of oxazolidinone antibiotics including linezolid, sutezolid, and delpazolid against clinical nontuberculous mycobacteria isolates. Regardless of macrolide resistance, for M. avium, M. intracellulare, and M. kansasii, sutezolid showed the lowest minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) value among oxazolidinone antibiotics. However, for M. abscessus, M. massiliense, the MIC and MBC values fo r all oxazolidinone antibiotics showed similar values. Oxazolidinone antibiotics warrant further investigation as potential antibiotics.

Multidrug Resistance Efflux Pumps in Acinetobacter spp.: An Update [Minireviews]

Acinetobacter spp. have become of increased clinical importance as studies have shown the antimicrobial resistant potential of these species. Efflux pumps can lead to reduced susceptibility to a variety of antibiotics and are present in large number across Acinetobacter spp. There are six families of efflux pumps that have been shown to be of clinical relevance: the Major Facilitator Superfamily (MFS), Small Multidrug Resistance (SMR) family, ATP-binding cassette (ABC) family, Multidrug and Toxi c Compound Extrusion (MATE) family, Proteobacterial Antimicrobial Compound Efflux (PACE) family and Resistance-Nodulation-Division (RND) family. A lot of work has been done on understanding and characterizing the roles that these efflux pumps play in relation to antimicrobial resistance and the physiology of these bacteria. RND efflux pumps, with their expansive substrate profiles, are a major component of Acinetobacter spp. antimicrobial resistance. New discoveries over the last decade have shed a lot of light on to the complex regulation of these efflux pumps leading to greater understanding and potential of slowing the reduced susceptibility seen by these bacterial species.
