In Reply Casale et al highlight our findings, which demonstrate that a transcanal totally endoscopic approach to middle ear and attic cholesteatoma is an effective alternative to postauricular surgery in children. We agree that the endoscope is one of many adjuncts that can facilitate successful removal of cholesteatoma. These adjuncts can be categorized as those that facilitate visualization, improve access, or eliminate invisible or inaccessible disease.
