The impact of the COVID-19 pandemic on the provision of surgical care
The coronavirus disease 2019 (COVID-19) pandemic has brought about unprecedented adaptations in healthcare management, be it at a local or global level. With so much that is not known about the disease itself, it has become manifestly clear that even less is known about how healthcare systems can contain such a disease. In this situation we are obliged as clinicians and academics to share our experiences in order to pool together vital information that will not only help to tackle the matter at hand...

A fistulating incarcerated incisional hernia: a case report
AbstractAn incisional hernia is usually a defect in the scar of an abdominal surgery. The natural history is intestinal obstruction with the risk of strangulation. We report a case of a long-term conservative management of an incisional hernia with an abdominal corset. This resulted in fistulation from pressure necrosis that required an en-bloc excision of the incarcerated fistulating bowel with the hernia sac. The defect was managed using the Jenkin’s ‘mass closure’ technique with no recurrence...

Giant phyllodes tumor occupying the whole breast in a young female: feasibility of mastectomy and immediate reconstruction
AbstractGiant phyllodes tumors are rare fibroepithelial breast neoplasms typically >10 cm by definition. The best investigation for preoperative diagnosis is core biopsy, although it is often difficult for the pathologist to distinguish fibroadenomas from phyllodes tumors and determine the level of malignant potential. The wide excision is the appropriate therapy for phyllodes tumors taking into consideration that mastectomy may be required for huge phyllodes tumors. In the modern era of breast...

Uniportal subxiphoid bilateral removal of self-introduced thoracic foreign bodies
AbstractThis is a report of a 31-year-old male refugee, who was admitted to Intensive Therapy Unit after being found in severe chest pain after escaping extreme torture from his home country. He was found to have four nails in his thorax. These were removed using a subxiphoid video-assisted thorascopic surgery (VATS) technique. This technique allowed excellent visualization of the right, left and anterior mediastinal part of the chest and therefore preventing damage or injury to surrounding structures....

Synchronous primary lung and breast carcinoma removed via a single incision
AbstractWe describe a case of a middle-aged female who was diagnosed with synchronous primary lung and breast cancer following a bout of recurrent chest infections. Subsequent Multi Disciplinary Team (MDT) discussion proposed that in light of the patients’ multiple comorbidities, both lesions should be resected simultaneously under one general anaesthetic. The patient underwent an initial left mastectomy and axillary node clearance. Through the same incision, a left anterolateral thoracotomy was...

An avulsion fracture of the anterior cruciate ligament attachment to the lateral femoral condyle in an elderly patient: a rare finding
AbstractAn anterior cruciate ligament (ACL) rupture is a common injury. The ACL usually tears in its mid-substance, an avulsion fracture of the ACL mostly occurs at the tibial attachment. In few cases, an avulsion fracture occurs proximally from the ACL attachment to the lateral femoral condyle. This is a rare finding, especially in elderly patients. We report a case of an avulsion fracture of the ACL attachment to the lateral femoral condyle after a traumatic injury in a 60-year-old female. Injury...

Micro-computed tomography analysis of the relationship between root canal number and root concavity in maxillary first and second molars in a Japanese population
Abstract Purpose The aim of this study was to investigate root canal configurations in maxillary first and second molars from a Japanese population, as well as the relationship between the root canal number and root concavity depth, using micro-computed tomography. Methods Maxillary first (N = 74) and second (N = 70) molars from a Japanese population...

Report From the International Society of Urological Pathology (ISUP) Consultation Conference on Molecular Pathology of Urogenital Cancers: III: Molecular Pathology of Kidney Cancer
Renal cell carcinoma (RCC) subtypes are increasingly being discerned via their molecular underpinnings. Frequently this can be correlated to histologic and immunohistochemical surrogates, such that only simple targeted molecular assays, or none at all, are needed for diagnostic confirmation. In clear cell RCC, VHL mutation and 3p loss are well known; however, other genes with emerging important roles include SETD2, BAP1, and PBRM1, among others. Papillary RCC type 2 is now known to include likely...

Cortical complexity and gyrification patterns in Parkinson’s disease
This study aimed to investigate the cortical complexity and gyrification patterns in Parkinson’s disease (PD) using local fractional dimension (LFD) and local gyrification index (LGI), respectively. In a cross-sectional study, LFD and LGI in 60 PD patients without dementia and 56 healthy controls (HC) were investigated using brain structural MRI data. LFD and LGI were estimated using the Computational Anatomy Toolbox (CAT12) and statistically analyzed between groups on a vertex level using statistical...

Neuroprotection of netrin-1 on neurological recovery via Wnt/β-catenin signaling pathway after spinal cord injury
The neuroprotective effects of netrin-1 after spinal cord injury and its specific molecular mechanisms have not been elucidated. In our study, Western blot, transferase UTP nick end labeling staining and immunofluorescence staining first showed that netrin-1 significantly decreased the expression levels of caspase-3, caspase-9, transferase UTP nick end labeling-positive neurons, nuclear factor kappa-B, and tumor necrosis factor-α after spinal cord injury, which inhibited neuronal apoptosis and inflammatory...

X-ray Dark-Field Radiography: Potential for Visualization of Monosodium Urate Deposition
Objective The aim of this study was to evaluate the potential of x-ray dark-field radiography for the noninvasive detection of monosodium urate (MSU) crystals as a novel diagnostic tool for gout. Materials and Methods Contrast-to-noise ratios of MSU crystals in conventional radiography and dark-field radiography have been compared in a proof of principle measurement. Monosodium urate crystals have been injected into mouse legs in an ex vivo experimental gout setup. Three radiologists...
