Updates in Antineutrophil Cytoplasmic Antibody (ANCA) associated Vasculitis for the ENT surgeon
Abstract ENT involvement is common in ANCA‐associated vasculitis (AAV), particularly in GPA and EGPA. Early recognition and treatment is important for good outcomes, yet evidence suggests that UK ENT surgeons may not consistently recognise the early features of AAV, despite a similar incidence to vestibular schwannoma. AAV is a rapidly advancing field, with significant developments in the understanding of its pathogenesis, classification and treatment over the past decade. Relevant vasculitis mimics...

Sequential Bilateral Implantation in Older Children: Inter Implant Map Differences and their Effects on Functional Outcomes
Abstract Objectives It is well established that sequential bilateral implantation offers functional benefits in speech in noise and sound localisation, although it can be challenging to get long‐term unilateral users to adapt to the second implant. The aim of this study was to investigate programming differences between the two cochlear implants that can impact on performance outcomes. Design Cohort Study Participants Sixteen older children who received sequential implants...

Delayed failure in tympanoplasty: A 20‐year study of the incidence and reasons for re‐perforation in 359 patients
Abstract Success rate in tympanoplasty can deteriorate with time after initial graft healing. Early successful repair of tympanic membrane does not necessarily guarantee permanent closure. It might be prudent to have long‐term follow up after tympanoplasty in a select group of patients. Potential risks factors for delayed failure include a continued smoking history, bilateral perforations and tympanosclerosis at time of surgery or its development post‐operatively. The...

Current opinion in otolaryngology and head and neck surgery: clival chordoma and its management
Purpose of review Chordomas of the clival region are challenging tumors to treat due to their location and infiltrative behavior. The primary goal of treatment remains a gross total resection (GTR). Advances in surgical techniques, especially the adoption of endoscopic endonasal techniques, provide improved rates of surgical resection while minimizing secondary surgical morbidities. Recent findings Advances in the management of clival chordomas include further development of endoscopic endonasal...

Purpose of review To describe and popularize the transmanubrial osteomuscular-sparing approach (TOSA) outside its original thoracic surgical field of application, based on the consideration that it could be of interest for the management of a number of head and neck cases in both elective and emergent scenarios. Recent findings The main advantages of TOSA compared with transclavicular techniques are its superb exposure of anatomic structures located at the level of the cervicothoracic junction,...

Controversies in the surgical management of sporadic medullary thyroid carcinoma
Purpose of review Medullary thyroid carcinoma (MTC) represents a wide spectrum of tumours with differing biology, behaviour and natural history. The only current available curative treatment is surgery in the form of thyroidectomy with or without ipsilateral or bilateral neck dissection. There is a lack of consensus in the available published guidelines on the optimum extent of initial surgery, and there is significant variation in clinical practice. This review focuses on the most recently published...

Airway management, symptom relief and best supportive care in anaplastic thyroid cancer
Purpose of review Anaplastic thyroid cancer (ATC) is a rare but aggressive form of thyroid cancer that is associated with significant morbidity and mortality. Because ATC is locally invasive, airway management is a critical component of treating these patients. Timely decisions regarding airway interventions can contribute to symptom relief and supportive care for patients. Over the last decade, there has been a paradigm shift in our recommendations for airway management. The purpose of this review...

Abstract Hidradenitis Suppurativa (HS) is a chronic inflammatory dermatosis in which B cells play a prominent but unclear role. Our understanding of the role of B‐cells in innate and adaptive immunity (including antibody production, antigen presentation and effector functions) is rapidly evolving; and these novel findings requiring integration into the pathophysiologic model of HS. Murine dorsal hair type is genetically determined by polymorphisms in candidate genes that influence BMP and WNT signalling
Abstract Mouse dorsal coat hair types, guard, awl, auchene and zigzag, develop in three consecutive waves. To date, it is unclear if these hair types are determined genetically through expression of specific factors or can change based on their mesenchymal environment. We undertook a novel approach to this question by studying individual hair type in 67 Collaborative Cross (CC) mouse lines and found significant variation in the proportion of each type between strains. Beauty is in the Eye of the Beholder and the Skill of the Surgeon
It wasn’t very long ago that it seemed like every woman walking around the fancy shops in Manhattan looked eerily similar to every other one: they were all proudly sporting the same nose! In 1957, the Goldman tip was popularized. It is a vertical dome division endonasal approach that narrows the tip of the nose and causes a slightly upward projection. This results in a pert, adorable nose popularized by many American television stars in the 1960s and therefore coveted widely. The Diamond nose has...

Cochlear Implant Reliability: Reporting of Device Failures
Abstract The aim of this study was to investigate the adequacy of the reporting of cochlear implant device failures. Data from a parallel study involving over 6300 children [1] was used to calculate the instantaneous failure rate for explantations. We found that this is comparable to what manufacturers term ‘Cumulative Failure Percentage’ (CFP). This finding raises concerns about the information provided by manufacturers on the reliability of their implants.
