Assessing Physiological Stress Responses to Word Retrieval in Individuals with Aphasia: A Preliminary Study
Objective: Word retrieval difficulty can be distressing for individuals with aphasia. However, physiological stress responses to word retrieval are relatively unstudied. This study aims to measure the physiological response (i.e., heart rate and respiratory rate) and perceived stress in individuals with aphasia during a naming task to understand whether word retrieval may be a stressor to them. Method: An aphasia group (n = 7) and a healthy group (n = 38) participated in 4 experimental conditions,...

Pure aortic valve regurgitation: SAVR is the gold standard, but TAVR is another gun
Key Points Surgical treatment is the gold standard for pure aortic regurgitation (AR). In this study, transcatheter aortic valve replacement (TAVR) appears to be a promising option for inoperable/high‐risk patients suffering pure AR. Long‐term results and specific patient‐level data are needed to better clarify the role of TAVR in pure AR treatment.

A step forward in predicting anterograde crossing success for infrapopliteal chronic total occlusions
Key Points Forty to fifty percent of critical limb ischemia patients have infrainguinal chronic total occlusions, frequently below‐the‐knee. With probabilities ranging from 21 to 90%, the Infrapop‐CTO Score presented here in CCI predicts the likelihood of anterograde crossing of infrapopliteal chronic total occlusions (CTO). Based on cap morphology, occlusion length, calcification, and restenosis status, the score is easily assessed in the lab. An unfavorable Infrapop‐CTO Score of 5 or...

Choose to fly higher rather than pacing around
Key Points Pre‐existing right bundle branch block and the depth of the prosthesis in the left ventricle are predictors of permanent pacemaker implantation (PPI) following transcatheter aortic valve replacement. The recapturable capability of the Evolut R system allows higher and more precise valve implantation and this was associated with halved incidence of PPI as compared with the CoreValve. Larger studies are needed to assess the feasibility of systematic very high implantation (depth...

BATMAN—Savior or vigilante?
Key Points BATMAN is a new technique to prevent left ventricular outflow tract (LVOT) obstruction from transcatheter mitral valve replacement (TMVR) by deploying the transcatheter heart valve from the apex through a perforation of the anterior mitral valve leaflet. The risks of uncontrolled balloon dilatation of the anterior mitral valve leaflet include extension of the tear superiorly into the aorto‐mitral curtain or laterally to avulse the trigone from the annulus. Percutaneous laceration...

Invasive imaging of myocardial infarction patients: Is less or more better?
Key Points Advanced intravascular imaging or fractional flow reserve in acute myocardial infarction patients may be safely performed and may be associated with clinical benefits. Since this is a nonrandomized registry with too few numbers to adequately evaluate mortality, it is uncertain whether clinical outcomes are truly improved. Adequately powered randomized trials using advanced imaging versus angiographic guided percutaneous coronary intervention (PCI) would be helpful.

Radial access: So much progress but a way to go
Key Points Transradial access is the key bleeding avoidance strategy in percutaneous coronary intervention. This study showed a marked adoption of transradial access over a 10‐year period, however, women had a significantly lower rate compared to men and overall bleeding events did not decrease over time. Strategies to overcome barriers to radial access in women and to maintaining competency in femoral access in all patients are needed.

Key Points Evaluation of the severity of aortic stenosis in the presence of dynamic left ventricular outflow (LVOT) obstruction is challenging. Tangled wire in a Dacron band during Cardioband transcatheter tricuspid annuloplasty—How to solve the problem
Abstract Transcatheter tricuspid valve intervention (TTVI) is an emerging treatment for patients with symptomatic severe tricuspid regurgitation (TR). Currently, the issues that arise during TTVI are unclear. Here, a 78‐year‐old woman with severe TR underwent transcatheter tricuspid valve annuloplasty using a Cardioband (Edwards Lifesciences, Irvine, CA). We attempted to cinch the Dacron band after anchoring; however, it was impossible to connect the distal tip of the size adjustment tool and the...
